Patients on repeat medication will receive a computerised printout with their prescription for ordering future medication. Please ensure that you use this form whenever possible as it makes it easier for us to manage your requests promptly.

Please order your repeat prescription in plenty of time by ticking the items you require on the order form and placing it in the reception box, handing over to the receptionist or by posting it to the surgery. You can also request prescriptions via our practice fax. Our fax number is 01246 208221. Please remember to include your name, date of birth and a full description of the medications including the name of the medication and dosage required. If you are requesting medication that has not been prescribed by the surgery before then you must provide details alongside your request.

The surgery based at Calow is a dispensing practice for patients living beyond a one mile radius from a chemist. Please ask about our over the counter medicines.
