Our lists are currently open and you may apply to register with us. We undertake not to discriminate against any application on grounds of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation.

We may, however, refuse your application to join us if:

  • You or your relatives have a history of violence or threatened violence or we believe that there is a real risk to the personal safety of practice staff or to the property of the practice.
     
  • You have previously been removed from the practice list, for example due to an irreconcilable breakdown in your relationship with a member of the practice team.
     
  • You are outside the practice boundary.

To complete your registration, you will need to complete a registration form together with a short medical questionnaire.

The Receptionist will give you a Practice handbook giving details of the Practice and the services we offer. This also contains the opening times of our two surgeries and details of how to book an appointment.

If you have an existing chronic disease (diabetes, asthma etc) you will need to book an appointment with one of our Nurses.

Please note: If you are on any regular medication, you will need to book an appointment with one of the GPs before we can prescribe this for you. Please bring all your regular medication to the appointment.
