Private medical examinations/reports can be arranged for insurance, travel, employment and driving purposes. A scale of fees is available from the Administration Team/Receptionist.

RIGHTS AND RESPONSIBILITIES

The aim of our Practice is to provide a Service of High Quality Professional Care for our patients and to care for each other. This is achieved by working together. Please help us to help you.
You have the right:

  • To be greeted in a welcoming and caring manner
  • To privacy and confidentiality
  • To be treated with courtesy and respect by members of the staff
  • To see your health records, subject to any limitation in law
VIOLENT and ABUSIVE BEHAVIOUR

The practice operates a zero tolerance policy towards violence and reserves the right to remove from the registered list, patients who are violent or abusive towards staff or other patients or who wilfully damage practice property or the property of other patients. In those circumstances patients will be advised how they can access General Practice Services. They may be required to use special, more secure, GP service specifically designed for violent patients.

Everyone working in the NHS has a legal duty to keep information about you
confidential and secure. You may be receiving care from other people such as Social Services, and we may need to share information about you to make sure you get the best care. However, this will only be done if there us a genuine need for information to be shared and this will always be kept confidential. If you want to see your own health records, please ask us. You are entitled to receive a copy, but a charge will be made. In certain circumstances your right to see some details in your health records may be limited in your own interest. The NHS publishes a range of leaflets giving more details about patient information which is available from the information Governance Manager at Derbyshire County Primary Care Trust.
